I know this post is from quite awhile ago - but I found a library that has a copy of what you're looking for - that is of course, if you havent found it already.
On the Small Animal Publications page, its article #99:

It says on their Library Services page, that most articles can be copied/scanned for email, so I suppose it wouldnt hurt to contact them & ask?
